A generalized rough list of the various negative perks with brief descriptions.

**Insanity: The end result of many different afflictions. Upon reaching, the player has passed all means of reasoning and will need to be restrained or killed to protect all those around them.

*Drug Addictions: Usage of various forms of drugs- whether by the players choice or not- will cause them to become addicted. Further levels of addiction can result in death
-Nicotine:
-Alcohol:

*Mental Addictions: Actions taken by the player can cause them to become addicted to certain feelings and if not heeded with caution upon discovery, will take precedence over the current adventure.
-Sexual Deviancy: Sexual urges can distract the player upon seeing something they're attracted to. The further the levels of sexual deviancy reach, the more the will be attracted to and the less they will be able to control their urges.
-Sadism: The desire to cause pain. While initially related to monsters, the player will become increasingly violent towards everything that moves, eventually attacking party members.
-Masochism: The desire to feel pain, which can result in the player removing their own armor or simply not attacking. The final levels nearly guarantee death.
-Glutton: The desire to consume everything. This can include player rations, items, monsters, and anything else edible. Can result in cannibalism.

*Fearful: Phobia's that can affect the players resolve. Upon a successful roll, the players fear will turn to anger. A failure can cause the player to panic.
-Fear of Monsters: Fear of certain monsters. Can be overcome with further interactions and successes.
-Fear of Darkness: Fear of treading dark places.
-Fear of Pain: Fear of being hurt in battle, lowering roll chances in battle.
-Fear of Divinity: Fear of the Gods and divine aspects.
-Fear of Failure: (Questionable)

*Mental Afflictions: Different mental ailments that, if left unchecked, can evolve into crippling negative effects.
-Sadness: Affects confidence and success rate of certain actions. Can evolve into Depression, drastically reducing success rates.
-Paranoia: Affects sanity and success rates of certain actions. Can evolve into Delusional, increasing violent actions towards NPC's
-Abusive: (Questionable)

Maybe a list of physical Negatives

Feeble- Strength stat only counts as half of what it truly is in the rolls.
Breathless - Endurance stat down
Clumsy- Dex/Agility down(not sure which)
Mutilated - missing a limb

Something bestowed you with a terrible curse! (Curse should be a spell right? Let's make it a spell.) Now you have this neg perk on you. What's the curse, exactly? Who knows! Maybe your skin is forever pink, maybe you can only see in complete darkness, maybe you had a stat drop to unacceptable levels, sdrawkcab etirw uoy ebyam... It could be anything terrible and permenant and completely character raping and rp enforcing! Be scared!

Death Touched - The physical body is dead and rotting away while the soul is still attached.

Tainted blood
You share the blood of a monster, be it being born from alien experiments or a werewolf bite that wasn't enough to take someone all the way to the lycanthrope stage or kill him.
You get some of the monster's attributes including personality, physical and magical capabilities and looks mixed up with the normal attributes of your race.
I'm not writing the exact effect because it depends on the monster and character race.

Deformed
You somehow have a deformed limb or body section which limits your physical handling.

Feline Heat
Intimate desires wake in yourself and the other sex on a periodic routine. While you'll make those of the same gender as you jealous to you it's a curse, as the other gender pursue you with seemingly uncontrollable lust; strangers in the street would invade your most private space, family meetings are as intense as they are awkward and long time friendships suddenly demand benefits you never wanted or thought possible.

Cannibal
You eat people.
+ damage vs your own race and subraces, -[All interactions with living cretures]

No luck of the Uncool
The odds of a bird shitting on you or finding money in the street are very extremely much low for you.
Situations influenced, dictated and made feasible by luck don't apply to you.
Example: On a natural 20 you can no longer stick your sword into your enemy's neck and drink the stream of blood that run down the blade. Instead you simply do your extra crit damage. That's because you're just not cool anymore.

Misperception
It's a bird? It's a plane? No, it's the neighbour's dog!
You always confuse sensory cues from you surrounding.

Misanthrope
You hate everyone and everything. The leafs that fall in autumn and the old couple two blocks from you can go to hell together with everything else.

Misophone
"-People using their car breaks to the point they squeak deserve to die. Also, the next time I hear someone turning a book page I kill myself."
Sounds instill you with extreme levels of physical repulsion, rage or depression.

Close minded
Because who needs new ideas when there're old ideas?
Effecr would be something like gaining less skill points on a level up, or something.

Discoordination
You're very goofy and clumsy.

Archaic Curse
A person touched by the archaic curse destroy everything s/he touch. Unlike the practice of archaic arts the archiac curse hasten the effect of the passage of time by the real course of time of those processes, not by previous damage or wear.
Archais curse is a condition, not an actual magical curse and is extremely rare. It does not affect the one with the condition and water.
It's curable via complicated magic, requiring a powerful magic user who study the Archaic arts and a few assistants or several more moderate-level practitioners to replace the powerful magician.
Many magic oriented and powerful groups and individuals who practice magic will want to get their hands on someone with this condition for academical purposes, as it allegedly provide insight to an untapped form of the archaic arts.
The truth:

Parasitic persona
An intelligent force is latched to your body and\or soul, wanting to use you to do things you don't want.

Meticulous
Your learning and research capabilities have greater potential but take longer to complete.
Effect: +Int but learning&research take 30% longer. ***Maybe requirements are higher (from a pc platform POV) or research (for spells and such) actually takes longer (from D&D POV), also apply to in-character riddle/puzzle solving skills (like if you need to roll for your guy to understand a puzzle, he'll get a stat bonus on it but it'll take him an extra round).
***I'm writing this as a negative because game mechanics-wise it puts a cooldown on your skills, which is always annoying from player perspective.

Looking Back at You
Reflections of creatures cause you to retaliate with fear, the thought that something that might, or perhaps even worse - might not be there and is looking back at you terrifies you.

So... I guess OCD could practically supply varied negative perks?

Firecatcher - You are induced with obsessive attraction to fire. Having this mental inclination means you get burned way more than you should and your skin suffers from it. -1[physical defences]

Battlescarred - Various marks  from old encounters spot your body giving you a less pleasant look.

Unbeliever - Buffs and heals recieved from divine sources are only half as effective.

Albino - You get penalties when you stay in the sun for long periods.

Soft - Your gentle way makes it hard for you to oppose someone who's being mean and unfair to you. - to persuasion and intimidation + to empathy.

Four Eyes - You require visual aid to see properly. To compensate for your shortcomings however, or in the process of causing them, you developed your skills; gain advantage either on listening or concentration that involves looking at something from up close for long durations.
